Advisor, Health Financing (Ethiopia)

The Role At Last Mile Health (LMH), we work to deploy a health worker within reach of everyone, everywhere. Do you have experience shaping health financing policy, influencing government decision-making, and building partnerships that strengthen national health systems? If so, you may be our Advisor, Health Financing with the Ethiopia program. The Advisor, Health Financing, will provide strategic technical support to the Federal Ministry of Health to strengthen sustainable financing for Ethiopia's community health system. Embedded within the Ministry's Health Strategic Affairs Department, the Advisor will support evidence-informed policymaking, advocate for increased domestic investment in the Health Extension Program (HEP), and work closely with government stakeholders to strengthen health financing policies, budget planning, and accountability mechanisms. Reporting to the Deputy Country Director, with a dotted line to the Director of Health Financing for technical mentorship, the Advisor will serve as a key liaison between the Ministry of Health (MoH), the Ministry of Finance (MoF), the Legislature, Regional and District Authorities, and development partners. Working as part of Last Mile Health's multidisciplinary Health Systems Strengthening team, the role will help ensure community health priorities are reflected in national policies, budget decisions, and long-term strategies that strengthen and sustain government-led primary health care. The role will contribute to Ethiopia’s broader efforts to strengthen primary health care (PHC) financing and support regional initiatives such as Africa Frontline First, which promotes the expansion and professionalization of community health workers across Africa. This position is a 24-month Fixed-term role with the potential of renewal. This position is only open to candidates who are currently located and authorized to work in Ethiopia. This role is onsite and located in Addis Ababa, Ethiopia. What You'll Do Domestic Resource Mobilization (DRM) for Health Extension Worker Program Analyze domestic resource mobilization trends for PHC and the HEP, identifying fiscal space options at federal and regional levels Develop an investment case for sustained government financing of HEP, targeted at Federal MoH, MoF, and regional health bureaus. Conduct cross-country comparative analysis of Universal Health Coverage (UHC) performance and health financing composition among Sub-Saharan African peer countries to generate evidence for Ethiopia's DRM strategy. Convene an annual high-level advocacy event with key decision-makers to drive continuous political prioritization of community health financing. Develop an annual community health financing brief summarizing funding needs, priority investments, and expected service delivery outcomes timed to the federal budget cycle. Explore and document feasible innovative financing mechanisms for community health in the Ethiopian context. Sample and analyze several woredas to identify challenges inhibiting budget allocation to community health programs at the district /Woreda level. Participate directly (Budget advocacy) in the budget-making process at select woredas and the national level to secure budget allocations for community health programs. Build the capacity of regional and woreda health offices in budget negotiation and budget advocacy. Develop at least 2 Health Financing policy briefs or research papers for national and cross-country use. Focus on topics such as: Ethiopia’s UHC performance comparison, service delivery components contributing to UHC, health financing composition vs. peer countries, the relationship between public health expenditure and UHC, financing/policy reforms, and required resource mobilization efforts Participate in LMH cross-country convenings and contribute Ethiopian lessons to the shared learning agenda. Pooling & Alignment of Funds Conduct comprehensive resource mapping of all financing flows supporting community health across government, donors, woredas, and implementing partners. Analyze financing gaps and inefficiencies across woredas and service delivery areas. Coordinate between the Ministry of Health, the Ministry of Finance, and development partners to align financing with national community health priorities. Develop policy recommendations to improve the alignment of partner financing with national community health strategies and government planning processes. Produce a financing fragmentation dashboard to identify geographic and programmatic duplication or underfunding across woredas. Assess the financial viability of Community-Based Health Insurance schemes at the woreda level, identifying drivers of scheme deficit and bankruptcy and options for consolidation Establish a regular community health financing review forum at national and select woreda levels, bringing together government institutions, woreda health teams, and development partners. Assess to establish the Health Extension Worker (HEW) services covered in the national health insurance and community-based health insurance Lead advocacy activities for the inclusion of uncovered HEW services into national health insurance and community-based insurance schemes. Support implementation of the revised Health Care Financing Strategy (HCFS) Track and contribute technical inputs to the PHC re-envisioning financing reform package, ensuring DRM strategy and HEP investment case are aligned with the emerging national PHC reform architecture Support the MoH in the progress review of the implementation of the revised Health Care Financing Strategy (HCFS) Identify bottlenecks and establish solutions in support of the implementation of the HCFS Conduct regular field visits to assess the progress of HCFS implementation Monitor and produce a brief progress report on the implementation of the HCFS What You'll Bring Bachelor’s degree in Public Health, Economics, International Development, Political Science, or related field. Progress years of experience in Economics, Health Economics, Public Policy, Health Financing, or Public Health, in partnership with government counterparts or other policymakers Professional work experience in health policy, health financing, Public Financial Management, health economics, and health systems strengthening is required; experience working in Ethiopia is an asset. Experience and knowledge related to the technical components of health economics and health financing (e.g., costing, cost-effectiveness analysis, Domestic resource mobilization, strategic purchasing, national health accounts exercises, resource mapping, etc.) Experience working with governments in a technical advisory role, especially supporting development of advocacy materials, communicating impact, policy analysis, research or other communications materials for policymakers. Quantitative analysis experience and skills in health economics and health financing space Demonstrated experience in health policy analysis in low- and middle-income countries, or resource-limited settings is preferred You'll Impress Us If: You are mission-driven with a commitment to health and social justice.
