Position Summary Impact Health PC is seeking an experienced Medical Director and/or Attending Physician to provide care within Skilled Nursing Facilities in the Hartford, CT area . This position offers the opportunity to provide direct patient care while also serving in a clinical leadership capacity. The Medical Director works closely with facility leadership, attending providers, nursing staff, and interdisciplinary teams to promote high-quality care, regulatory compliance, and continuous clinical improvement. Physicians may have the opportunity to serve as an Attending Physician, Medical Director, or a combination of both , depending on experience, availability, and facility needs. Key Responsibilities Provide high-quality medical care to residents within Skilled Nursing Facilities. Serve as an Attending Physician and/or Medical Director based on facility needs. Provide clinical leadership and medical oversight to support quality patient care. Collaborate with facility leadership, nursing staff, advanced practice providers, specialists, and interdisciplinary care teams. Review patient records, clinical outcomes, adverse events, infection control initiatives, and quality metrics. Participate in and provide oversight for Quality Assurance and Performance Improvement (QAPI) initiatives. Ensure care is provided in accordance with applicable federal, state, and facility regulations. Provide evidence-based recommendations regarding diagnoses, treatment plans, and overall patient management. Maintain appropriate on-site presence and remain available for clinical consultation as needed. Arrange appropriate physician coverage when unavailable. MD or DO from an accredited medical school. Active, unrestricted Connecticut medical license. Board Certified or Board Eligible, preferably in Internal Medicine, Family Medicine, or Geriatrics . Current state and federal prescribing registrations, including controlled substance authority as required. Eligible to participate in Medicare and Medicaid programs. Skilled nursing, long-term care, geriatrics, hospital medicine, or similar clinical experience preferred. Knowledge of long-term care regulations, quality standards, and clinical operations. Strong leadership, communication, and organizational skills.
