Overview Physical Therapist, General — Clearlake, CA. Travel assignment starting 08/03/2026 for 13 weeks. Weekly pay range: $2,095 to $2,151 per week. Guaranteed hours: 36 per week. Shift: 3x12-hour days. Work setting: outpatient rehabilitation within a critical access hospital. This position supports outpatient therapy with minimal inpatient coverage as needed. Key Highlights - Weekly pay: $2,095–$2,151 per week - Guaranteed hours: 36 hours per week - Contract duration: 13 weeks - Shift: 3x12-hour days - Start date: 08/03/2026 - Setting: outpatient rehabilitation in a critical access hospital Responsibilities - Provide physical therapy evaluation, treatment, and re-evaluation for patients with disabilities, injuries, or diseases, aiming to relieve pain, restore motor function, and maintain optimal performance per APTA standards. - Develop, implement, and adjust individualized treatment plans based on physician orders and patient goals. - Document patient progress, treatment plans, and outcomes in the medical record according to policy. - Collaborate with physicians, nurses, and other health professionals to coordinate care and optimize patient outcomes. - Educate patients and families on home exercise programs, safety, and long-term wellness strategies. - Provide occasional inpatient coverage as needed, in addition to outpatient duties. Requirements - Education: Bachelor’s Degree from a four-year accredited college or university in physical therapy, or degree in a related field. - Experience: One to two years related experience and/or training, or equivalent combination of education and experience. - Licenses/Certifications: - Current California Physical Therapy license, or eligible for registration - Current BLS certification - Valid California driver’s license when travel is required - Additional: Member of APTA preferred; Able to work independently with minimal supervision; Strong communication and organization skills.
