Overview Physical Therapist, General — Tillamook, Oregon, United States. Start date July 27, 2026. 13-week travel assignment with guaranteed hours of 36 per week. Shift: 5x8 Days. Setting: Critical Access Hospital. Employment: Travel - Temporary Assignment. This role requires in-home care experience and involves mileage Reimbursement. Key Highlights - Weekly pay: 2,241 to 2,268 per week - Guaranteed hours: 36 per week - Contract duration: 13 weeks - Shift: 5x8 Days (day shift) - Start date: July 27, 2026 - Travel assignment in a Critical Access setting - Mileage reimbursement included - Home health/home care experience noted in the job scope Responsibilities - Plan and conduct segments of treatment programs to restore and improve physical, social and mental functions, tailored to each patient. - Perform evaluations and establish individualized evaluation and discharge plans; communicate plans to the patient, family, and care team. - Provide therapy interventions using established protocols and techniques appropriate to the patient’s needs. - Develop and document comprehensive clinical plans, ensuring timely and accurate records within established deadlines. - Supervise assigned assistants and coordinate with the interdisciplinary team to optimize patient outcomes. - Maintain clear, professional communication with patients and families, providing education and progression updates. Requirements - Education: Doctor of Physical Therapy (DPT) degree; required, only if license was obtained on or after January 2016; or foreign equivalent. - Licenses/Certifications: Physical Therapy licensure in all applicable states of practice; CPR certification or Basic Life Support (BLS) from an approved vendor. - Driver’s License: Valid driver’s license; must be at least 21 years old. - Experience: At least two years of recent experience in the field; current BLS certification; and a current nursing license per the state of assignment. - Additional: Must have experience in Home Care/Home Health, and must have experience with Home Care Home Base and PointCare software.
